Step 1: Basic Business Information (Lines 1–7)

In this section, you need to enter your basic LLC details.

  • Enter your LLC name exactly as registered in Line 1. If you have a different business name, you can enter it in Line 2, otherwise leave it blank.
  • For the mailing address, use your registered agent address or business address. Make sure it is correct because the IRS may send your EIN letter there.
  • In Line 6, enter the county and state where your business is located.
